Disability Assessment and Coordination Entity for GOHSEP

The buyer seeks a Disability Assessment and Coordination Entity to develop a statewide plan for identifying and assisting people with disabilities during disasters. The contractor will conduct needs assessments, provide training, coordinate stakeholders, and deploy teams to shelters. They will maintain engagement activities, provide documentation and analytics, and ensure appropriate referrals are made. The goal is to build and sustain an active network of resources to support people with disabilities before, during, and after disasters.

- Develop a statewide plan for identifying and assisting people with disabilities during disasters
- Conduct individual assessments of needs for people with disabilities in shelters and disaster areas
- Provide training on meeting disaster-related needs of people with disabilities
- Coordinate with disability stakeholders and government agencies to enhance cooperation
- Deploy team to shelters to conduct onsite assessments and provide referrals
- Maintain engagement activities to improve services and supports for those with disabilities
- Provide documentation and reports on activities performed before, during and after disaster events
- Create and maintain analytics on DACE Team activities and referrals
- Collaborate with advocacy groups to engage health and social services providers
- Ensure referrals are appropriately matched to needed resources and services
