• Provide case management for 90 individuals at two SRO facilities
• Conduct information gathering and problem assessment for clients
• Develop self-sufficiency goal planning and service coordination
• Facilitate on-site programming on substance abuse and life skills
• Maintain client records and prepare monthly reports
• Meet quarterly with Housing Authority staff
• Develop and maintain linkages with other support services
• Provide advocacy and referral services for clients
• Evaluate client progress toward self-sufficiency
• Obtain information on family or other support systems for clients
