Population Health Learning Collaborative Facilitation and Management

The buyer seeks to establish a contract for management services of a statewide learning collaborative to promote best practices in heart disease and stroke prevention. The collaborative will take a comprehensive, health equity focused approach to prevention and management of hypertension and high blood cholesterol, with a focus on identified priority populations in New Mexico. The contractor will facilitate and manage the collaborative, including recruiting participants, planning meetings, evaluating strategies, and reporting on outcomes.

- Establish and support a statewide Population Health Learning Collaborative
- Recruit community organizations, health systems, and social services to participate
- Develop milestones, MOUs, and incentive structures for the collaborative
- Plan and facilitate quarterly meetings and annual conference
- Maintain collaborative webpage and regular communication with participants
- Evaluate strategies to ensure health equity and social determinants of health are addressed
- Conduct quality improvement activities like Plan Do Study Act cycles
- Provide monthly invoices, progress reports, and annual report on deliverables and outcomes
