The buyer is seeking comments on changes and additions to requirements for a new Support Coordination External contract. This contract will replace the expiring DHS90828 contract. The buyer aims to update timeframes, modify activity log requirements, implement new tools, and revise visitation and service request processes. The buyer is also looking for feedback on outcome measurements and other proposed changes throughout the Scope of Work.

• Provide support coordination services for individuals with disabilities
• Develop and implement Person-Centered Service Plans
• Conduct regular face-to-face visits with clients
• Assist with caregiver compensation processes
• Ensure proper site certification and licensing
• Update employment and vocational information in USTEPS
• Complete activity log notes within 15 calendar days
• Utilize approved Person-Centered Planning tools
• Implement the Employment Pathway tool for eligible individuals
• Manage budget-neutral service changes within plan totals
