Buyer seeks a provider for Information and Assistance Program services for older adults, adults with disabilities, and family caregivers in Los Angeles County. Services include telephonic information, needs assessment, referrals, and follow-up. Provider will also assist with disaster planning for older adults. Buyer requires 5 years of relevant experience and specific Project Manager qualifications. Services to be provided throughout Los Angeles County excluding the City of Los Angeles.

• 1/8/2025 - Mandatory Proposers' Conference
• 1/8/2025 - Written Questions Due
• 1/17/2025 - Questions and Answers Released
• 1/24/2025 - Proposal Due Date

• Minimum of 5 years experience in past 7 years providing IAP or similar services
• Project Manager with required qualifications and certifications
• Not debarred by any government agency in last 5 years
• Current, valid, and active Unique Entity Identification Number
• Organizational structure of non-profit, for-profit, public/government entity, or joint powers agency

• Provide telephonic information and assistance to older adults, adults with disabilities, and family caregivers
• Assess individuals' needs and link them to local services
• Provide referrals to qualified programs
• Follow up to ensure individuals have obtained services
• Work on disaster planning and preparedness activities for older adults
