• Must be a health department operated by state, county, city, or tribal government

• Implement policies and systems to increase cardiac rehabilitation referral and participation
• Support delivery of virtual cardiac rehabilitation sessions
• Develop strategies to improve access for priority populations
• Generate and disseminate data on cardiac rehabilitation use
• Educate eligible patients about cardiac rehabilitation benefits
• Establish partnerships to improve cardiac rehabilitation referral and completion
• Link patients to supplemental social support services
• Participate in regular project update meetings
• Join and contribute to the Public Health CR Partnership
• Measure project success through process measures and aligned metrics
