Buyer seeks an effective, integrated developmental screening data collection and management solution to support the Follow Along Program. The system must use ASQ-3 and ASQ:SE-2 questionnaires and be configurable to meet State needs. Buyer requires a user-friendly platform available online and via mobile application for completing questionnaires, receiving developmental guidance, and facilitating timely referrals. The solution should reduce inefficiencies in accessing program data for analysis and reporting.

- Provide integrated developmental screening data collection and management solution
- Implement ASQ-3 and ASQ:SE-2 questionnaires with fidelity
- Set up configurable/customizable COTS solution to meet State needs
- Create user-friendly online and mobile platform for completing questionnaires
- Facilitate timely referrals to needed services and supports
- Conduct system testing including security and performance tests
- Perform accessibility testing against WCAG and Section 508 standards
- Provide training for internal MDH FAP program staff and MNIT support staff
- Offer ongoing licensing, solution support, maintenance, and hosting
