• Provide homeless outreach services using multidisciplinary canvassing teams
• Build rapport with unsheltered homeless individuals and families
• Connect homeless with emergency shelter, permanent housing, and critical services
• Provide urgent non-facility-based care
• Assist with mail handling
• Provide civil legal services to help access permanent housing
• Use Housing First approach to rapidly access housing options
• Assist with obtaining documents for housing applications
• Help obtain income through public benefits and employment
• Provide support through housing location and access processes
