Buyer seeks proposals for an Early Start Infant Development Program serving up to 50 children aged 18-36 months. The program will provide multi-disciplinary services primarily in natural environments. Buyer requires a provider to develop and operate a community-based program aligned with DDS Rate Reform model. The provider must employ qualified staff, implement evidence-based practices, and collaborate with interdisciplinary partners. Services should be ready by January 1, 2027.

  • Director must have bachelor's degree and 18 months management experience or 5 years human services experience
  • Supervisor must have 3 years human services experience or bachelor's degree in related field
  • Proven history of supervising or providing services to target population
  • Understanding of Person Centered Practices and IPP process
  • Experience developing and operating similar projects
  • Valid California driver's license and no criminal convictions
  • Meet Title 17 CCR standards for Community-Based Day Programs and Infant Development Programs
  • Develop multi-disciplinary Infant Development Program for up to 50 children aged 18-36 months
  • Provide intervention activities promoting physical, cognitive, language, psychosocial, and self-help development
  • Implement evidence-based practices and individualized approaches for effective service delivery
  • Employ qualified staff including OT/R, PT, Speech Pathologist, and BCBA
  • Maintain ADA-compliant office/clinic in HRC catchment area
  • Develop training plan for child development intervention curricula and techniques
  • Utilize valid screening tools and provide timely progress reports
  • Collaborate with multi-agency, interdisciplinary partners
  • Align services with DDS Rate Reform model for service code 805
  • Develop specific individual outcomes through IFSP process
