• Regularly engaged in operating a recuperative care site for at least 3 years
• Current Medical Respite/Recuperative Care contract with Alameda Alliance for Health
• Submitted authorizations and claims for Medical Respite
• Possess all necessary permits, licenses, and professional credentials

• Operate the Fairmont Medical Respite site as recuperative care
• Ensure medically appropriate interim or permanent housing upon exit
• Provide nursing and caregiver services for complex physical health conditions
• Connect clients to medical home and transport to critical appointments
• Coordinate services with community-based case managers and County medical staff
• Provide wound care, medication support, and injection medication assistance
• Offer behavioral health services including counseling and psychiatric services
• Manage site including janitorial, maintenance, and security services
• Collect and report data on program services and outcomes
• Maintain 90% nightly unit occupancy rate
