The buyer seeks proposals from qualified agencies to provide High Fidelity Wraparound Preventive Services. The services aim to avert disruption of families and prevent placement of children in foster care. The buyer utilizes a Solutions Focused Principles-Trauma Informed Care framework and incorporates principles of Diversity, Equity, and Inclusion. The program targets families with children under 18 at risk of out-of-home placement.

• 3/17/2025 - Proposal Due Date

• Experience providing Preventive and/or High-Fidelity Wrap services to target population
• Ability to perform background checks for all staff
• Documented experience implementing evidence-based practice with fidelity

• Provide High Fidelity Wraparound services to minimum 60 families annually
• Maintain 1:10 caseload ratio for Care Coordinators
• Conduct Child and Family Team meetings at least monthly
• Develop and implement individualized Plans of Care
• Provide 24-hour on-call support for crisis situations
• Document case activities in CONNECTIONS and Fidelity EHR systems
• Coordinate with Vendor Network to provide targeted support services
• Participate in monthly Utilization Management meetings
• Maintain fidelity to High Fidelity Wraparound model and principles
• Collect and report on required performance measures and outcomes
