The buyer seeks Home Delivered Meals Service Provider(s) to prepare and deliver nutritious meals to homebound seniors aged 60 and older in Genesee County. The provider must have capacity to deliver two meals per day, at least 5 days per week, that meet dietary guidelines for older adults. Services include intake, assessment, meal preparation and delivery, and coordination with other agencies. The contract is for one year with option for two one-year renewals.

• Must have capacity to provide Home Delivered Meal Service 7 days a week
• Must deliver meals at least 5 days a week
• Must provide 2 meals per day (3 if client receives liquid nutritional supplement)
• Staff must have satisfactory criminal background check
• Staff must have negative tuberculosis test at time of hire

• Prepare and deliver nutritious meals to homebound seniors aged 60 and older
• Provide two meals per day that meet dietary guidelines for older adults
• Deliver meals at least 5 days per week, with availability 7 days per week
• Offer modified diets and liquid nutritional supplements as prescribed
• Conduct intake, assessment and referrals for meal recipients
• Maintain records of all meals served and client information
• Coordinate with other agencies to avoid duplication of services
• Survey and document client satisfaction with meal services
• Develop and implement an emergency preparedness plan
• Conduct background checks and TB tests for staff and volunteers
