The buyer is seeking qualified practice transformation organizations to provide practice transformation services for large scale projects. The selected organizations will work together to provide support to primary care, behavioral health, and potentially specialty practices to improve health and healthcare in Colorado. Services include practice facilitation and clinical health information technology advisor (CHITA) services.

• 2/25/2025 - Optional Pre-Proposal Conference
• 3/5/2025 - Last Date to Submit Written Questions
• 3/17/2025 - Proposal Due Date

• Provide practice facilitation to healthcare practices
• Provide clinical health information technology advisor (CHITA) services
• Assist with collection of required assessments for each practice
• Facilitate practice change to achieve project-specific milestones
• Participate in PTO learning community activities
• Coach practices using standardized approach based on project goals
• Support HIT aspects of practice transformation milestones
• Help practices develop sustainable processes for clinical quality measure reporting
• Identify HIT best practices and disseminate lessons learned
• Document practice progress via project-specific field notes
