Buyer seeks proposals for Adult Residential Facilities with Social Rehabilitation Treatment Services. Buyer requires providers to offer stable and structured therapeutic programs in unlocked, home-like settings. Providers must comply with state regulations and work with buyer to facilitate discharge plans.

• Be a non-profit, for-profit organization or other legally constituted business entity
• Have current Medi-Cal Certification or ability to become Medi-Cal Certified
• Be a Certified Long-Term Adult Social Rehabilitation Residential Facility
• Have California Department of Social Services Community Care License as Adult Residential Facility
• Have certification with California Department of Health Care Services as Social Rehabilitation Residential Facility
• No unsatisfactory performance with government agencies in past 24 months

• Provide adult residential facilities with social rehabilitation treatment services
• Operate stable and structured therapeutic program in unlocked, home-like setting
• Offer long-term (up to 18 months) or transitional (up to 12 months) treatment services
• Teach and enhance skills for independent living
• Facilitate discharge plans to move clients to lower levels of care
• Comply with California Code of Regulations Title 22, Title 9, and Community Care Licensing requirements
• Bill county monthly in arrears for services provided
• Maintain fiscal, statistical, and management records for at least 10 years
• Obtain required insurance coverage
• Register in County's Electronic Procurement Network (ePro) system
