The buyer seeks a qualified vendor to provide a Human Services Case Coordination System. The system will replace the current one used by the Riverside County Office on Aging. It must be a comprehensive, multifunctional SaaS solution for data collection and care management. The system should support various programs and services, including MSSP, and meet NAPIS and CARS requirements. It must provide case management tools and a secure I&R data management system.

• Provide Software as a Service (SaaS) case coordination system with minimal customization
• Implement client tracking system for NAPIS and CARS requirements
• Develop fundamental case management tools
• Create scalable and secure Information and Referral (I&R) data management system
• Incorporate central web-accessible I&R data repository through URL development
• Support Resource, Content, and Consumer groups of data in I&R database
• Replace current system utilized by RCOoA
• Facilitate coordination of services from various agencies and organizations
• Support services for Area Agency on Aging, Older Americans Act, and Older Californians Act programs
• Enable implementation of complex, multi-agency service plans
