• Recruit healthcare systems into Unite Ohio network
• Recruit community-based social service organizations to Unite Ohio
• Link Unite Ohio referral data with clinical data in HIE
• Track referrals for individuals with cardiovascular disease risks
• Provide technical assistance to CVH QIP practices
• Generate disaggregated data by priority populations
• Submit quarterly referral and utilization reports
• Create annual reports on data linkage outcomes
• Participate in monthly progress meetings
• Develop solutions to overcome implementation barriers
