- Recruit healthcare systems into Unite Ohio network
- Recruit community-based social service organizations to Unite Ohio
- Link Unite Ohio referral data with clinical data in HIE
- Track referrals for individuals with cardiovascular disease risks
- Provide technical assistance to CVH QIP practices
- Generate disaggregated data by priority populations
- Submit quarterly referral and utilization reports
- Create annual reports on data linkage outcomes
- Participate in monthly progress meetings
- Develop solutions to overcome implementation barriers
