Buyer seeks facilitation and consultation services related to the Title V Maternal and Child Health Block Grant (MCH Block Grant). The contractor will organize meetings, manage performance monitoring, provide consultation on Action Plans, and complete a comprehensive Needs Assessment by 2030. Services include facilitating educational opportunities, coordinating Domain Leads and Partners meetings, and developing surveys and data presentations. • Assist with Domain Leads and Partners Meeting preparation
• Manage performance monitoring for the five domains

• Attend annual MCH Block Grant Site Visit
• Complete comprehensive Needs Assessment by July 2030
• Facilitate Listening Sessions for community input
• Develop and distribute constituent surveys
