- Conduct outreach events at least four times monthly in high-need areas
- Establish intake protocols using county-provided standardized process

- Support One-Stop-Shop events with setup and onsite assistance
- Coordinate meal planning and food distribution at community events
- Collect and maintain client demographics and service utilization data
- Submit quarterly invoices and biannual progress reports
- Canvass monthly to promote One-Stop-Shop event attendance
- Provide referrals for individuals with chronic disease and mental health disorders
- Identify and engage with homeless encampments and underserved communities
- Participate in monthly coordination meetings
- Develop educational materials aligned with county standards
- Protect confidential client information from unauthorized use or disclosure
- Target individuals experiencing transportation barriers
- Seek county approval before distributing outreach materials
- Collect client health and social data through assessments
- Coordinate donation collection and distribution at events
- Track program performance and identify improvement areas
- Provide feedback on service barriers and potential solutions
